Important Consent I, the parent/guardian of the above-named player, a minor, agree that I and the player will abide by the rules and regulations of the Somali Community of Minnesota youth soccer program. In consideration of the players participation in the soccer programs and activities of the Somali Community of Minnesota youth soccer program. I, for myself and the player in our respective heirs, administrators, and successors, intend to be legally bound here by the release and indemnity of the Somali community of Minnesota youth soccer program. I further grant the S.C.O.M parties the right to use a player's name, picture, to broadcast and other material concerning the programs provided that are related to the players status as a participation in the programs.
